To update the Texas Department of Insurance (TDI) TexasHealthOptions website information about individual accident and health plans, TDI requests that carriers complete a survey about the individual accident and health products they currently offer in Texas to new customers. The link to the TexasHealthOptions website is http://www.texashealthoptions.com.
Please submit your survey using TDI's secure web application. The attached instruction sheet explains how to submit your survey. The filing is due no later than May 31. We make this request pursuant to Section 38.001 of the Texas Insurance Code.

- Send an email to Jack.Evins@tdi.state.tx.us and request the link to the survey application on TDI's secure website.
- Both the Username and Password are your TDI company number. If you do not know your TDI number, please look it up at https://apps.tdi.state.tx.us/pcci/pcci_search.jsp
- After you log in the first time, the system will prompt you to create a unique password. Please be sure to make a note of the new password.
- In your survey response, include only information about policies or coverages you are offering to new customers.
