Agent and Adjuster Licensing: Name, Address and Contact Information Updates

Licensees must notify TDI immediately regarding changes to contact information, to your legal name, or your business’ name or address (Texas Insurance Code § 4001.252 and 28 Texas Administrative Code §§ 19.902 and 19.906).

Different types of changes, as outlined below, must be submitted using different means.  Some changes require a fee.

LDTL = License Division Trade Name/Location

NIPR = National Insurance Producer Registry

Changes for Individuals

Contact Information Change

Individual agents and adjusters can update most contact changes online, such as: a change of address, email, phone or fax numbers through the National Insurance Producer Registry (NIPR) system for contact change requests.  NIPR reports these changes to TDI.   Begin your contact change request by visiting the National Insurance Producer Registry and selecting the ‘Start’ button. There is no fee for this change request.

Individual agents and adjusters may also update contact information such as: official mailing address, official resident address, business address, email address, or daytime phone number, by submitting a signed version of the Licensee Name/Address Change Request (TDI Form FIN533) to TDI. There is no fee for this change request. Please review the instructions on the form carefully to ensure all necessary information is included, and that any delivery or mailing instructions are followed.

Legal Name Change

Legal name changes of licensees are not allowed electronically and must be reported by submitting a signed version of Licensee Name/Address Change Request (TDI Form FIN533). FOR INDIVIDUALS ONLY – A supporting official court document (e.g. marriage certificate, divorce decree, or other official court document) is required to be submitted along with this form. (Driver licenses or social security cards are NOT acceptable supporting documentation).  There is no fee for this change request.

In addition to name changes, you can also update an individual licensee’s official mailing address, official resident address or business address using this form, with no fee applicable. (See 'Contact Information Change' above)

State to State Address Changes (other than Texas)

State to state address changes are not allowed electronically and must be reported by submitting a signed version of Licensee Name/Address Change Request  (TDI Form FIN533).  There is no fee for this change request.

Transition from Nonresident License to Resident License (Texas)

A nonresident licensee who becomes a resident of Texas will need to submit the Application for Residency Change to Texas (TDI Form FIN594) to change a nonresident license to a resident license.  The form requires the following: change of address information; a Letter of Clearance from the current resident state (applies to agents only); and background information, including fingerprints. There is no fee for this change request.

If a licensee's prior resident license has been inactive for more than 90 days, the nonresident agent will need to apply for a new license using the Application for Individual Agent License (TDI Form FIN506), and pay any applicable fees.

An individual, changing residency to Texas, is subject to requirements as stated in TIC Chapter 4001, including any financial responsibility and fingerprint requirements.

Changes for Businesses/Entities

Changes for businesses or entities must be reported using the forms listed in the sections below.

Business Address Changes (other than Texas)

Businesses must submit a signed version of Licensee Name/Address Change Request  (TDI Form FIN533) to change their address.  There is no fee for this change request.

Transition from Business/Entities Nonresident License to Business/Entities Resident License (Texas)

Businesses must submit a signed version of Licensee Name/Address Change Request  (TDI Form FIN533) to change their address.  There is no fee for this change request.

A business/entity, changing residency to Texas, is subject to requirements as stated in TIC Chapter 4001 (emphasis TIC §4001.106), including any financial responsibility and fingerprint requirements.

Official Name Changes for Businesses/Entities

If a currently licensed business or entity needs to change their official name or register an assumed name (any name other than a true name or present legal name), they must submit a signed copy of Registration of Assumed Name/Branch Locations/Entity Name Change  (TDI Form FIN528). FIN528 is commonly known as the LDTL Registration form, or License Division Trade Name/Location form. A filing fee applies.

Address Change for an Existing Branch

Submit a signed version of Branch Office Address Change Request  (TDI Form FIN532) to report an address change for an existing branch of a business or entity.  There is no fee for this change request.

Adding a Branch Office

To add a new branch office, submit a signed copy of Registration of Assumed Name/Branch Locations/Entity Name Change  (TDI Form FIN528), commonly known as the LDTL Registration form, or License Division Trade Name/Location form. A filing fee applies.
